Having a problem at set up. When I grip the club with my left hand, my left shoulder tends to go inward and my shoulders are offline. Any suggestions on getting the shoulders set up properly and online at address?

If the "inward" motion you describe closes your shoulders to the target line, this is fine. Better the shoulder line is slightly closed rather than open as it facilitates a forward swing direction from inside the target line. When viewing video of Moe, you will notice that his shoulders were generally closed to the target line at address, returning to square through impact.
